How to change US Dollars in an Indian Bank?

I have 5000 US Dollar and I am an NRI and now on holidays. If i would like to change this money from an Indian Bank, what are the formalities, what are they papers the Bank will ask for changing this money. If any one has changed money in Indian Banks, please let me know.

1. Take ur passport and any identity card with u to any Indian bank foreign exchange branch and change , that's it !!
